Location: North Chicago, IL
Schedule: Monday Friday, 8:00 AM 5:00 PM
Job Type: Permanent, Full-Time
Pay: $70 000 per year (DOE)

Overview:
Our medical practice in North Chicago is seeking a compassionate Registered Nurse (RN) to provide care to patients in their homes and communities. New grads and experienced RNs alike are welcome to apply.

Key Responsibilities:

• Conduct comprehensive nursing assessments of patients physical, emotional, and psychosocial status
• Develop, implement, and evaluate individualized care plans focused on symptom control, comfort, and quality of life
• Provide pain and symptom management, including medication administration, monitoring, and timely intervention
• Educate patients and families on disease progression, care options, medications, and what to expect at each stage of care
• Coordinate care with physicians, nurse practitioners, social workers, chaplains, aides, and other team members
• Perform home safety assessments and implement interventions to promote patient and caregiver safety
• Document assessments, interventions, and patient responses accurately and promptly in the electronic health record (EHR)
• Participate in interdisciplinary team (IDT) meetings, case conferences, quality assurance and performance improvement (QAPI), and in-services

Qualifications:

• Active, current Registered Nurse (RN) licensure in Illinois
• Strong communication and organization skills, with the ability to work independently while collaborating with an interdisciplinary team
• Travel throughout surrounding areas required; valid driver s license and reliable transportation required
