NoSubmission RequirementsALL MUST BE NOTATED AT TIME OF NAME CLEARBoard Certified in Oncology: REQUIRED Active State Medical License (Michigan): REQUIRED BLS: REQUIRED Clean Malpractice & License History: REQUIRED no exceptionsCopy of the NPDB, self-query requires at presentation (Should be pulled within 30 days) - REQUIRED
